Respected sir.
My patient hospital no is : 707846h name : nanda deb laskar and regd in your opd. After the consulting physicians has given us different medical tests and which we have duly carried out and after we have paid all required fees for reports and prescriptions on due time, we have not received the require reports from the concerned office as advised by the physicians.
Since, we have visited your respective office counters, the reports are not given to us and hence we have our flight tickets booked for 8th sept 2019, we have to come back without reports.
I, therefore request and pray for all the reports from the concerned departments
To be made available for further need in future and for which i have already paid.
