Complaint regarding non-receipt of Photo-identity card

We were living at Mahim and were voting at Bombay Scottish High school booth during 1990-95. We shifted our residence to Shivaji Park and filed necessary forms for change of address and filed the papers on 14/07/2009 with Electoral office(School of Los Angeles) for self, wife and daughter and obtained acknowledgement receipts.

My wife and daughter received their photo-identity cards and I have not received the same.

Today when I went to the polling booth with acknowledgement receipt, I was not allowed to vote as my name is not there in the voter list. I am disappointed that I could not cast my vote today. Same thing happened during Lok sabha elections.

Between July 14th and Sept 25th, 2009, I have gone there 10 times and our names have not been yet confirmed in the voter list. The electoral office staff said that they would be sending the new photo identity cards home. But I did not get my identity card.
It appears that in India, citizens cannot exercise their voting rights even if they take all necessary steps much in advance- three months before elections (I filled up our ‘change of address’ forms on 14/07/2009, submitted and got acknowledgement!!).

I was given the following numbers to make complaint and none of them answered the phone numbers: 24366493.

Sarnopat([protected]) – said that he had been transferred to Malabar Hills
Surve ([protected])- not answered – phone kept on ringing

On 25.09.2009, I approached electoral officers such as Ravi Patil, Gurjar at D’souza school but they were not able to get anything done by the insolent staff they have at the electoral offices. I am tired and I am giving up!

Father's Name is also Mismatch on it. I complained many times to District Election Office but there was no action taken by them.
