Election Commission UP varanasi - Voter ID Card

Dear sir
My mother have been issued with a Voter id card bearing No 94/1196AVD0122531 for 388 Varanasi north Shivapur constituency and still she has not received the card where as other family member of my family have received the card pls help me or suggest the proceedure to get the card.

My me and my family has file voter id form two tims for constituency 388 part no.165 Varanasi north Shivapur and still we not received the card please help me or suggest the proceedure to get the card.
