In my country, as a reciever I don't have to pay such approval fees, is the Bank require such charges.
Can you tell me on an urgent basis (before I sent the money) :-

2. Is this scam ?

My ATM card got stuck in Onipan branch ATM mechine on a Saturday, April 2009. Thus, I filled a transafer form to pick it up at Broad street branch being the nearest branch to my office. However, have gone to Broad street branch more that 4 times but only to hear the same old story that Onipan branch has not responded to my request.
