I was allotted shares vide Lr NO SBI/1092236 dtd[protected] & ledger folio 1838824, which were not received by me till date. plz advise me the on line / off line procedure for obtaining the share certificates and post me required documents on [protected]@gmail.com or contact [protected]. And also I have received the divident till 2004. Hoping for the urgent action.

My client was expired on 17.01.2009. After his death his wife and son had approaches me for issue of duplicate share certificate of 50 Shares of SBI Certificate No.1313910 Dist. No.[protected]. You had mentioned in your communication a number of form specified. kindly let me know from where these forms are obtained by the claiment. The claiment had clearly stated that the above No certificate has not been actually received by Late Gyan Chand Soni ( Form I, T, S, A Q&N ) are mentioned in the letter bearing No. DFSL/SE/DCL/TMN-DUP[protected]/65)/2014 dated 23/09/2014
