Voter ID — non receipt of voter's id

Dear Sir,

I have not received my voter id card.
I have submitted all the relevant documents to the representative since last 10 months ago.
And My Name also Appear in the booklet of the election Vote List. As my son Visited almost 6 times
to collect this Card, BUT ALWAYS refused by replying that it will comes by Post, but till date not received.
As we cannot visit to the Corporation Office to Collect this.As Me & My Wife are in the age of 71 & 68 years of Age.

Please provide some assistance as the parliament elections are comming and being
a responsible citizen I want to cast my vote.
