I have submitted Application for new Voter Card of my daughter, Miss Jivika Mittal at Dehradun Public School Campus at Sanjay Nagar, Sector 23, Ghaziabad in the month of November, 2008 to be received by us in January, 2009, but the same is not yet received. I was two times fillup form no.001C with photo and last time fillup form no. 001C at Barrackpur Administeration Bhavan of my wife Smt. SUJATA SARKAR for her voter identity card but till date not received her Voter identity card also note that, my EPIC no. and my wife EPIC no. are same in voter list and shown my photo against my wife name in voter list.

Please, rectify and send voter identity card of my wife.
