Voter ID Photo Card — Not received new Voter ID Card

I am applied for change of addresss in Voter ID photo card. But not received new photo voter id card.

I had applied for voter's ID through online during 2008, and they had come for verification also.
But till date I had not received the voter ID.

Where/whom should I Contact

dear sir i have already voter id card but today when i reached to pollling booth i found that my name was not in list although i have voter id card with photo. not only me but there is lot of persons wo faces same condition like me . on the other hand people says that every person should vote to make a stable government. where on the other hand what will do a lots of peoples who want to vote but have problems like this...
