Dear Sir,

My self Debdas Podder Insurance No-[protected] amounting Rs. 4700/- & Rs.3551/-till date I've not received my reimbursement amount which incurred for treatment . Detail below

Bill Submitted at SD Chandi chwok on dt: 16.01.2015 & 16.06.2015 and the same forwarded at ESI Sealdah Accounts Office on 22.04.2015 & 14.10.2015 after that ESI Sealdah division till not forwarded the same to ESI corporation 5/1, GRANT LANE, KOLKATA -700 012.
Kindly look in to this ASAP
