ESIC — Medical claim is not in transfer

Dear Sir, I am Kuldeep kumar My ESIC IP NO -[protected].I am working in company JAC Air services .My medical claim is not in transfer my account. Your Dwarka staff is very rough behave and give abusing. I go there maximum 20 times April 2015 to till date.
