ESIC — Not Yet Received my ESIC Card for IP no-[protected]

Hi, Actually i was employed in the ibm daksh, gurgaon where my esic ip no is-[protected] and my pehchan is done on dated-22/10/2010 and my card was dispatched to my mailing address:- argaghat road, makatpur, giridih-815301 but card was not delivered to my address and may be returned back.So at that time i was not made any complaint, because my salary enhanced and i was not eligible for esic.But now i came back to my hometown and joined company named-Saluja Steel & Power Pvt ltd where i got new esic ip no-[protected] in which my contribution get credited.Few days back my new IP no gets disable and previous no gets enabled.So i need my card for your reference my uhid no is-DL01.[protected] .Please resolve my issue to get my updated card as soon as possible.My email id- [protected]@gmail.com Thanks & Regards Guruwant Singh
