Ikya Human Capital Solutions — date of exit not update

Dear sir,
I m Jitendra Singh my employee code is A266142
i left the job but at a time for pf release it ask for date of exists.Sir i beg you to provide my exist date as it ask in umang app for pf release.my date of joning is 02/04/2015 and my date of exit is 15/12/2015. please help me I want withdrawal my pf. Please I m facing this issue long time.
