Poor management both at top level and mid management.
One can find a lot of nepotism around. Too many mid level managers who are "yes boss" type and do not challenge the status quo.
The culture in several teams is a stinker and it stems down from the vertical heads who are too young and immature to lead teams.
It is a standard practice by the top and the middle management to pass the blame below and fire employees basis "performance".
Too many people in managerial positions and having very little knowledge about the industry. People in the system for a long time are lackadaisical and have a lot of power.
Management pls think about the problem you are trying to solve and you will understand why certain things don't work.
Stop fooling yourself and other stakeholders through unreliable metrics. Some of the top management is hardly seen to be working.
It seems like they come to do some phone calls in the organisation. If you care about education and think it is a serious business, stop fooling learners and set clear expectations about what the program offers. Talk to mid management and the teams to realise the problems faced on the ground.
Try to get in more outsiders in the system especially at top leadership level to resolve the current mess.
