Sir, I JC-191670-F ex Sub Pirthi Singh applied for 64kb ECHS CARD on 05/09/2022 vide Regn No [protected] online. The is required to be verified by BEG RECORDS, KIRKEE. I am a heart patient and requires immediate treatment. But ECHS poly clinic has refused to issue temporary slip for want of service particulars by records.
