Nil — form 16a not given by the bank

Sir/madam
I have bank account inpunjab national bank nahlain bearing no [protected]. The bank has deducted income tax from my pension and fix deposited. This bank has not given form 16a so far. It is difficult for me to file return without form 16a. On the other hand when we approached before the bank authority they did not cooperated and misbehaved.
