not print application of conforme form

my all detail are complete fill on form and my conform no 31488426 but not print application print so please give revert on my email: [protected]@gmail.com

I am not going to count all errors and bugs, otherwise this form would be too short.
Simply I just want to say "If you can't handle the site, close it"

I am try last two day for registration for tat examination form but some error created so i am not submitted examination form, my old confirmation no[protected] and fee already paid when i submitted old form.
