State Bank Of India [SBI] — stop off auto renewal

My sbi general insurance policy was opened n august 2016. I didnt opted for auto renewal. And agent was fraud me as 100rs in year, while from my account debited 1300rs every year till date. How can auto-renewal possible without self-attested documents. I have to stop it with immediate action asap. I want my refund asap. I was busy in my job and work. I couldnt see debit money from my debited amount from my saving passbook bcz my bank account is 800km far from my home.
