United Bank Of India [UBI] — Not applying my ATM card after applying

Address:Nagaon, Assam. Pin-782001

I (Bhanu Devi) have already applied my ATM card on 21 11 2021 in United Bank Of India at Nagaon Assam.. Today on 11 02 2022 I got the information that they don't apply my ATM card, infact they don't accept my Kyc form..
So I request you to take an action against such carelessness work of the bank
