I am Superintendent, Kuntara CHC, Kuchinda, Sambalpur District, Odisha. I was provided BSNL CUG sim by NHM, Odisha bearing number [protected]. It was deactivated last year. I went to BSNL customer care, Sambalpur office but they said can't help.
