Being a kidney transplant patient, I have been prescribed your product Cyclophil ME 100 ORAL USP. I purchased this product in June 2014. The product has a bottle with the solution in it and a plastic inlet nozzle. This plastic in-let nozzle has fins around it made of poor quality of plastic as shown in the attached pictures. PF Transfer from previous Employer to Biocon
I was working with Dr Reddys labs ltd Hyderabad till 03.08.2007. I joined Biocon ltd at bangalore on 10.08.2007 and submitted form 13 for transfer of my PF amount from Dr Reddys to Biocon. The details of my PF account are:-

Land line not functioning
Dear Sir / Madam,
Kindly note that my friend's Mr Rajashekara T.S with tel No 2672 1630 in AGS layout is dead since 15 days. Further on Sat ie on 23rd I myself have booked a complaint with docket no 1506.
Till today there is no response. Further in Hoskerehalli exchange a lady gave me a tel no 2672 0999, for the same no body is responding.
We once again request conserned personnel to rectify telephone...
