Dear Sir,
My self Ravi Kumar gupta & my D.O.B. is[protected]. I am a policy holder of Max Newyork Life insurance ltd. co. & my poicy no. is[protected]. Due to some personal reason i am finding unable to continue my policy. So surrender my policy & make my full & final note.
plz do the needfull. i will be highly oblized.

I am J.M.Aggarwal, R/O 9a/1, Saini Colony, Model Town, Karnal applied for Life insurance policy with MAX NEW YORK LIFE INSURANCE vide their proposal Policy no.[protected]. Due to some personal reasons, I applied for cancellation of this policy vide my application dt.03/06/08, which was very well accepted by their Karnal office duly marked receipt. The cancellation was telephonically confirmed by their karnal office.
