Citibank — personal loan incorrect amount, incorrect tenor, incorrect interest rate

Hi, i have applying for personal loan for 1.5 lacs for 2years at the rate of interest rate is 11.49 as per talking with city bank loan officer. But when received my application i shocked because all details are mentioned are incorrect. It's my first time apply and very bad experience of applying for loan with city bank. The details are mentioned below.
