Columbia Asia Hospital — delaying my pf claim

I sumbitted my pf cliam three times at banglore kr puram pf office. But three times my pf forms and reauthrization form rejected because cash section not received my form. Then why my form rejected ??. Why again i submitt all forms its not a good job from pf kr puram office my pf account number PY/KRP/35841/9091.
