EFP — Waiting for EPF amount since from more than last six months

My EPF account number is: PU/PUN/0122066/000/0000437.

I have submitted form to retrieving my EPF amount on 4th Nov., 2011, however till date I didn't received and amount or any acknowledge regarding same. Also when I check my claim status through online it gives 'Record Not found' message.

Kindly request to you, to look into this and try to resolve this issue ASAP, since I need my amount on urgent basis.
