My Election Card no. is LWR0077594 on which the incorrect date of birth.

It is my sincere request to please change the same or else let me know the process for change.

You got to fill up Form ID and submit the same alongwith attested copy of Driving license/PAN Card/valid ID Card and two copies of your Passport size photographs(one of which is signed across) to the Electoral Officer
