While returning from office i lost my Voter Identity card. alawy they returned me back telling that they haven't recd the form. Pls look into the matter immediately. My card No. is ISU/0200096.

Sir,
While returning from office i lost my Voter Identity card. Same day i reported and an FIR was lodged in Police for the same. I went to election Commissioner office for duplicate card. alawy they returned me back telling that they haven't recd the form. Pls look into the matter immediately.My card No. is DVX/0415687 .
