PF amount not transferred

My current organisation on my behalf raised request to transfer Provident Fund from "EPF A/C NO. KN/BN/19214/069174" to "EPF A/C NO. HR/GGN/28371/4446". It has been about an year and till now the PF fund has not been transferred.
Claim Id:- GNGGN111000004672
I have personally followed up by going to the office and was being told that transfer will happen soon but no action till now.
