I, S.K. Jindal, aged 44 years, son of K.L. Jindal, residing at house no. 284, J.P. Colony, West Arjun Nagar, Agra- 282001, Uttar Pradesh, do hereby solemnly affirm and state as follow:
1.That above named company was at then date of an order of winding up the same viz the Kuber Mutual Benefits Ltd, day of

3. I deposited money in various Fixed Account promised by the Kuber Mutual Benefits Ltd that on the maturity of the account. The company give back the principle amount of money together with the interest with company depart. In the above circumstances the claim is preferred.
