Sir,

Most respectfully I beg to state that I have sent an application on 12 Jul 08 by Registered letter (RLAD A3490 dated 12.08.2008) to Branch Manager, Swaroop Nagar, Kanpur-208002 to transfer my Money Plus Policy to my Present above said address as I have taken discharge from Air Force and settled in my home town. The details of my Policy are as follows:

Policy No : 233942597 Date of Commencement : 08/05/2007
Plan & term : 180 05 05 Date of Maturity : 08/05/2012
Issuing Branch : LIC of India, CBO-5, 7/111-B, Swaroop Nagar, Kanpur-2008002

After completing of 4 months, they have not transferred my policy to my new address. Consequent on this I have discontinued my premium.

In view of the above, it is requested to transfer my policy immediately to Midnapore Branch (West Bengal) code- 466, so that I can get my money back from Midnapore branch after locking period.

I will be obliged for an earliest and immediate action.
