Pan Card — PAN card sent back to office due to change of address

Address:Guntur, Andhra Pradesh

Hello Sir,

I am Mahesh Kothamasu.I have applied for PAN card and it was not received to me due to change of address.After applying for the PAN we are happened to change the house and the address supplied for the PAN is different to that of we are living in.Hence give me suggestion on how to get back my PAN to me.
