1. Please refer to your sanction No Maturity/2948/APS-NM-117095-L/(key id 235171) dt 18 Apr 2011.

2. I, JC-762360L Nb Sub Krishan Kumar presently posted with MCEME Secunderabad. My Policy No APS-NM-117095 has been matured on 17 Apr 2011. The amount credited to as Rs 85392/- and the amount Rs 1488/-(Arrer premium incl interest ) has not been paid to me at that maturity time.

3. You are requested to issue payment order for Rs 1488/- in favour of FPO 610 at the earliest.
