I have applied as nominee of Late Basanti Nayak on her death on dt.28.3.2012 for claim against SAHARA UNIQUE for Rs.10,000/- vide account No.[protected] at Chandi Chhak, Cuttack Branch. I have received of Rs.14,606/- after TDS deduction of Rs.1152/- vide cheque No.544679 dt.14.6.2012 drawn on Bank of Baroda, Cuttack Main Branch. Hence, I require 16 A in respect of TDS against my PAN No.CZEPS6795M as per I.T. rule
