[Resolved]  Voters ID Card — wrong entries in id card this time even the name was not there

respected sir,
I have got the voters id card but unfortunatly the entries are all wrong and this time even my name was not there. I was not able to vote. it is very shame on the staf who sees the form and enter the name and address.They even dont know to see the filled application and write the correct information.now please tell me what should i do now whom should i contact to get the job done.
