With respect I Smt.Laxmi Bala Swain, Junior Clerk, O/O.CDMO, Nayagarh beg to state that My NSDL account no.[protected],

Most humble & respectfully I Smt.Laxmi Bala Swain, Junior Clerk O/O.CDMO, Nayagarh beg to state that since 6/11 to till date monthly subscription @ 10% of my Pay+G.P+DA has been deducting against my PRAN No.[protected].All the PRAN applicant has already been received their PRAN Transition Slip. But I have not received my provisional Accounts Statement slip against my deduction from the Authority concerned. My D.D.O.is the Asst.District Medical Officer(Medical)Nayagarh.
So I would request you to consider my above grievance & issue account slip accordingly. For which act of your kindness. I shall be highly obliged to you.
