Employee's Providents Fund Organisation [EPFO] — employer not attested claim form and not done kyc update

Sir my employer not attested claim form and he not update my kyc so sir i am suffering from big trouble and my uan account no.[protected] i have already claimed that time claim was settled but now i am not able to claim because that time i am mistakenly applied 10c but my account is still running contribution was deposited regularly i am complaint at epfo tinsukia they told give one application from employer regarding this issue but my employer refused to give application
So sir please solve my issue
