Form-19 has been rejected in twice mentioning the reason that date of exit is wrong. Date of exit was given earlier 19.07.2018 on the basis of medical separation date from the organization but on the basis of my last contribution deposited was up to 30.04.2018 and date of exit on 01.05.2018. After submission of my joint declaration on 23.04.2019 for correction of date of leaving and full father's name again i have submitted my claim but this has also been rejected for the same reason. This really very painful for me and for my treatment also.

Hi,
My UAN No is[protected]. I Have submitted Claim form 31 ..2 times but pf department rejected my claim without any reason.. My Mother Facing breast cancer now days so i want withdraw my pf in advance.
It is my request to you kindly help me to withdraw my pf amount.
