Employee's Providents Fund Organisation [EPFO] — joint declaration form submitted for correction of information

My co-worker Shri. Tarak Nath Maity having UAN No.[protected] submitted Joint Declaration Form to Kolkata EPF office on 12.12.2017 and 31.05.2018 respectively for correction of my Name, DOB and Telephone Number etc.

Correct Name will be Tarak Nath Maity instead of Tarak Maity, correct DOB will be 13.09.1969 in place of 13.04.1960 and correct Phone Number should be [protected] from earlier [protected].

But on inquiry he came to know that the correction have not done yet.
