Dear Sir,

My PF a/c no. is HR/GG/29206/0087. i have applied for refund on 01.11.2012, i received the payment on 27.11.12. My p.f. had deposited Rs. 1440.00 for September'2011 to April'2012, while Rs. 1022/- in June'2012 for May'2012.

The deposited amount is 13982.00 under Form 19& 10. While i have received 9676+4080 only.

You are requested kindly to pay me the difference at the earliest with interest.
