Sir,
With reference to the subject cited above. I Sri Pramod Kumar Achary beg to inform you that, when I proceed to the HP Gap Dealer (Asian Gas Service, Phulbani) on dated 17/01/2014, the dealer intimated that, I have availed total 12 Nos subside Gas Cylinders since 01/04/2014, But, as per my Gas Booking Pass Book & Online Booking history, I have actually availed 09 ( nine ) No’s of subside gas cylinder since 01.04.2014 to till date i.e. 25.12.2014. For which I am enclosing herewith the following support documents for your kind reference.
Owing to such situation, I have lodged a complain bearing reference No.14192186, dated 2/2/2015. Further, I came to know that this may be happend due to submission of Form-5 (Mandate for Opting Out of Subsidy and PAHAL DBTL Scheme) to the Gas Dealer, such problem has been arised. But, to the best my knowledge, I’ve never done this types of mistakes at any cost. On verification through online at my hpgas.in, it is found that I have been provided only 09 (nine) no’s of subside Gas cylinders by the date.
Therefore, I would request you kindly look into the matter personally and take necessary immediate action to retrieve my balance 03(Three) Nos. subside Gas Cylinder and for supply subsidy LPG Gas at an early date for which I shall remain ever grateful to you.
