Dear Sir/Madam, The IDBI policy numbered – [protected] was issued in favour of my mother Mrs. Pratibha Varshney on 30thJune, 2014. The details of original policy were as follows: · Insured Person’s Name: Pratibha Varshney · Proposer’s Name: Ravindra Kumar Varshney (changed by forgery to ‘Pratibha Varshney) · IDBI Federal Manager’s Name: Shanthi Yagyanath (Manager’s Code no. - [protected]) · Agent Name: [SELF] Bhumika Gupta (D/o the insured, Code [protected]) The forgery came into notice when I took a close look at the policy document (which I fortunately opened to look for the due date of premium renewal), in which FAKE SIGNATURES of the Insured and the Agent (myself) have been made at following places in the Proposal form and Amendment form: · Point No. 11 of ‘Proposal Form’ Part I. The occupation of the proposer has been scratched and changed from ‘House wife’ to ‘Employed in Private Sector’, with 2 fake signatures of the proposer. Also, the salary of the insured (housewife) has been changed from NIL to 2LPA!
