Income Tax Department — pan card issued in my name informed by banker through cibil report generated by bank which is not related to me

Address:10/54,B.D.CHAMBERS, SHOP NO 14, D.B GUPTA, ROAD, KAROL BAGH, NEW DELHI, New Delhi, Delhi, 110005

I am holding only one pan card no aafpk4275n in my name and address and many loans are running on this pan applied by me.
I had applied for new loan from bank last month and bank inform me that there are two pan cards in my name which was surprising and shocked me. Other pan no is aampk6568m for which i do not have any information that when it was issued and to whom.
Kindly take the strict action towards the person who is responsible for this card and cancel this card.
