Jindal Vijaynagar Steel Limited — to change the address having physical shares

I am in possession of 4 shares of jindal vijaynagar steel limited in physical form since 2005, since then i have not received any dividend/bonus/right whatsoever has been declared by company
I also want to know the procedure to change the address & same i want to dematerialised in my demat account
Please guide me with detail procedure for address change & document required to add to my demat account
