Respected Sir,
Reg: Non receipt of Domiciliary Treatment Benefit Claim - LIC's Health Plus Plan policy #[protected] dated 25/02/2008 of Mrs. Sujatha Radhakrishnan
The subject policy is in the names of Radhakrishnan S and Mrs Sujatha Radhakrishnan. Cataract was detected in both eyes of Mrs. Sujatha on 02/05/2022 and was advised to undergo operations immediately.
A mail intimation was sent to LIC on 10/05/22. Operations were done on 12/05/22 & 17/05/22. Claim was lodged on 23/05/22 under Domiciliary Treatment Benefit Claim on 23/05/2022.
Till date we have not received any response from LIC in this regard. I am a retiree and a senior citizen as well. Undue delay in this payment is creating problems for me.
I therefore humbly request you to issue necessary instructions to Divisional Office - 3 which is the servicing office.
