Naac Accreditation — fraud in student satisfaction survey form by the university

Respected sir,
I am a student of dr. Babasaheb ambedkar marathwada university.
A day before i got call from the university office asking me to send my student satisfaction survey form to them. When i didn't send them the form the hod of my college called me up and asked me to send them the student satisfaction survey form immediately and they said me that we are just doing it for the some sort of counting.
Now my form, which was unfilled and was not submitted but after i send my form to the university in a few minutes it was filled and submitted without my consent.
So a form which has to be filled by a student was now filled by a university staff and fooled the students so that the real truth for the teaching and learning is hidden.

Please pay you kind attention to this matter. A students right is at risk.
