Punjab National Bank [PNB] — I received my bank statement from one of my relatives who doesn't have access to my account

I received my Punjab National Bank Statement from one of my relatives who doesn't have access to my account or any authorization to do anything in my bank account. No money is deducted from my account but how someone else can get my bank statement without my permission. The statement has all the transactions from January 2021 till May 2023.
The PDF has A/c. Ledger Inquiry written at the top left corner and then their is a box in which all my details are their such as A/c ID, A/c Status, A/c Open Date, Available Amt. etc.
Then their are two pages in which all my transactions are their from January 2021 till May 2023.

Please look into this matter that how someone can access my bank details without my permission
