Sir I have submit my daughter tuberculosis treatment claim ref no.[protected] dt. 09.01.2020 and my member ID N90227702LICGKP. I am employee of LIC OF INDIA BO-2 GORAKHPUR UP. I had sent all query document for ct thorax and suporting investigation dt 05.11.2019 by our office latter dt.15.12.2020 and mail 25.02.2021.
