I narbesh kumar tara singh has admitted in civil hospital silvassa on 23rd january 18 due to "fracture left distal femur compound, ga type ii with dnvd" in an accident. Operation was done on 25 january 18 & discharge on 29th january 18. I requested the mlc report but given a letter from the hospital, which is not valid for the mediclaim. So i humbly request you again to please provide the mlc copy & “letter from treating consultant stating the intoxication details at the time of rta ".
