Tata Docomo — wrong data entered in records when sim card was purchased

My data was entered wrong at your end which includes name mismatch, father's name mismatch and date of birth mismatch. The husband's name was entered as the father's name. Because of this i am not being able to get a replacement sim card for my number. As my original correct data in my documents differ with the wrong data entered by your staff. I want an immediate solution to this.

Can I reactivate my Tata DOCOMO number after a long time of not using it even if it says that number is no longer valid?
