I, jayeeta chaudhury, resident of flat no 9, srishti apartment, 237 ho chi minh sarani, sakuntala park, kolkata-700061 had opened six seperate fixed deposits, five with mis facilities and one payable on maturity. Mis amounts i have received upto july 2015 since then i have not received either mis amounts and maturity amounts from the company in spite of several reminders personally at alchemist kolkata office and also through mr. Basudev manna of resident sakherbazaar, behala and agent of alchemist through whom all the fds were made through him. Details of certificates given below:-
