I Ajay Kumar Tiwari S/O Diwakar Tiwari R/O E7 Awas Vikas Colony Amethi (UP) 227405 State that my son/daughter, DOB as mentioned in Service Document is incorrect. same is required to be amended as per Educational Certificate /Aadhar Card of Tripti Tiwari 20/12/2005 Sristi Tiwari 20/03/2007 & Harsh Tiwari 20/06/2010.
2. Pl fwd the same at the earlies for my future/carried.
3. Sir I am facing great difficulty, i request to you, please rectified my complain at the earliest.
4. I humbly request you sir to look into the matter personally so that I can live my remaining life peacefully.
